Plazomicin's mechanism of action involves binding to the bacterial 30S ribosomal subunit, inhibiting protein synthesis and leading to bacterial cell death. It specifically targets the aminoacyl-tRNA site (A-site) on the 16S rRNA, causing misreading of the mRNA and preventing proper protein translation. This mechanism is similar to other aminoglycoside antibiotics, but plazomicin is less susceptible to inactivation by certain aminoglycoside-modifying enzymes (AMEs) due to structural modifications. Detailed Mechanism:
- 1. Binding to the 30S Ribosomal Subunit:Plazomicin, like other aminoglycosides, binds to the 30S ribosomal subunit, specifically the A-site of the 16S rRNA.
- 2. Interference with Protein Synthesis:This binding disrupts the normal function of the ribosome, interfering with the correct translation of mRNA into proteins.
- 3. Codon Misreading:The altered ribosome structure causes misreading of codons, leading to the incorporation of incorrect amino acids into the growing peptide chain.
- 4. Mistranslation and Cell Death:The resulting faulty proteins and impaired protein synthesis ultimately disrupt essential bacterial processes and lead to cell death.
- 5. Resistance to AMEs:Plazomicin's structural modifications, including the 6'-hydroxyethyl group, make it less susceptible to acetylation by certain aminoglycoside-modifying enzymes (AMEs) like AAC(6')-Ib, which are common resistance mechanisms.
- 6. Rapid Bactericidal Effect:Plazomicin exhibits rapid bactericidal activity, meaning it effectively kills bacteria at relatively low concentrations.
- 7. Clinical Use:Plazomicin is approved for the treatment of complicated urinary tract infections (cUTI), including pyelonephritis, caused by susceptible bacteria, including Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Enterobacter cloacae.
